Where is the clinic?

Alex works from a home office in Bellevue Hill Sydney. The address and parking recommendations are outlined in the welcome letter sent out upon booking your appointment.

Are online or phone consultations available?

Skype, Zoom and phone consultations are available to those that cannot travel to Bellevue Hill. These are also available to overseas clients. Should this be your preference please let Alex know upon booking your appointment.

How do I pay for the session?

Payment is only required at the end of the session and are GST free. Alex currently accepts cash, bank transfer and EFTPOS payments.

What is the cancellation policy?

Cancellations are accepted with 48 hours notice. This allows Alex to schedule another patient in from the waiting list. Less than 48 hours notice will incur a charge of 50% of the consultation fee.

Are private health rebates available?

As a Nutritionist I am a registered provider with a number of private health funds, which means you may be able to claim a rebate (depending on your level of cover). The following health funds cover nutrition: NIB, Medibank Private (includes AHM), Teachers Union, CBHS and Australian Unity (AU). Please check with your health fund to see if you have the appropriate cover.

Where do I send the questionnaire and previous medical results?

Please send all questionnaires and medical documents from the last 6-12 months to info@alexandramidleton.com.auat least 2-3 days prior to the session.

How do I reorder my supplements?

Most supplement regimes require review every 1-4 months. If there is no repeat on any Vital.lyprescription you want to continue or have run out of, please email Alex to enquire at alex@alexandramiddleton.com.au and she will endeavour to reply to you as soon as she can.

What is the process with the GP referral form?

In the initial appointment Alex will write for you a GP referral letter to request specific assessments (blood/urine/scans/etc) that may help eliminate any potential cause of your current health issue. Please take this letter with you to the GP appointment along with any Douglass Hanly order form Alex also writes for you on the day. Please ask the GP to make sure Alex is copied in to the results as she is registered with most NSW laboratories. Lastly, if the doctor refuses to order any of the assessments (this is rare) please email Alex to let her know so she can organise a private lab to conduct them.

What do I do if I have a question to ask Alex?

If you have any questions about your assessments and treatment plan, please email Alex at alex@alexandramiddleton.com.au and she will endeavour to reply to you as soon as she can. In regard to the time taken to complete tasks outside of appointments, any paperwork required will incur a fee and will be charged as per the hourly rate. This includes: email consultations, treatment plan modifications, letters, report analysis, etc. Alex will inform you as required to ensure your approval is sought prior to beginning the work.
